Time-limited, structured support for patients returning home after hospitalization, facing a new serious diagnosis, or experiencing sudden functional decline. Dr. Prather helps families understand what happened, what has changed, and how to plan confidently for what comes next.

  • Reconciliation of medications, follow-ups, and new diagnoses
  • Family meetings to plan realistically for the months ahead

An ongoing consultative relationship with Dr. Prather for patients with stable or moderately complex needs. Provides regular, periodic expert guidance as health evolves over time. Visits may be in person or by video.

  • Continuity with a physician who already knows your story
  • 6-month or 12-month enrollment — 5% discount for annual
