Prather Medicine provides physician-led, consultative specialty care for patients whose complexity exceeds the time or structure of a standard clinical visit.
Written consultation summaries are provided following every encounter. Clinician-to-clinician communication is available when clinically appropriate.

- Diagnostic clarity for cognitive or memory concerns, including dementia staging
- Comprehensive geriatric assessment for medically complex older adults
- Serious illness consultation focused on symptoms, goals, or decision-making
- Longitudinal palliative care consultation outside of a formal hospice program
- Support interpreting prognosis or navigating post-hospital care transitions
- Time-intensive family conversations not feasible in standard visit structures
- Patients with high decisional complexity, functional decline, or caregiver distress

- Geriatric consultative evaluations (one-time or ongoing)
- Palliative care consultations for serious illness, non-hospice
- Care transitions support following hospitalization or new diagnosis

- Primary care
- Hospice or a hospice-affiliated program
- Team-based palliative care
- Geriatric case management or care coordination
We welcome referrals from primary care physicians, specialists, and other clinicians. Simply share our contact information with the patient or family — they can reach out directly to explore whether we are a good fit. We are also happy to hear from you directly when you have questions about a potential referral.
