What We Do
- Evaluate memory and cognitive changes with honesty and thoroughness
- Provide a big-picture medical perspective for older adults with multiple conditions
- Provide age-friendly geriatric assessments to help individuals age their best
- Help families understand what a diagnosis means—and what to realistically expect next
- Offer palliative care support for serious illness without requiring hospice enrollment
- Provide medical clarity after hospitalization, when everything feels uncertain
- Deliver written care summaries with clear recommendations after every visit
- Communicate with your existing doctors to ensure continuity
What We Are Not
- Not a primary care practice—we work alongside your existing doctors
- We do not replace your primary care physician or other specialists
- Not hospice and not affiliated with any hospice program
- Not a care management or case management service
- Not available for urgent or emergency medical situations
- Services are not billed to Medicare or private insurance
How It Works
- All new patients begin with an Initial Assessment (60 min + 30-min follow-up)
- You receive a written care summary and recommendations after every visit
- Visits may be in person, by video, or in your home
- After your Initial Assessment, you may enroll in a longitudinal care plan
- Enrollment is time-limited and structured to respect your time and investment
About the Cost
- Prather Medicine is a direct-pay practice—no insurance is accepted
- Services cannot be reimbursed by Medicare
- This model allows longer, more thoughtful visits without insurance constraints
- All fees are transparent and shared before any commitment is made
- A 5% discount applies to annual longitudinal care enrollment
- Payment details and options are discussed during the intake process
